Thank you for choosing Ring Beyond for your VoIP solution. Your current service provider requires this letter as a proof that you have explicitly authorized and requested that your current telephone numbers and services be transferred to another service provider. By filling in all the information requested below, and signing this letter, you provide Ring Beyond and their agents with the authorization to become your new primary agent for the purpose of provisioning voice services and initiating the process of transferring your telephone numbers away from your current carrier. Please do not cancel any telephone services until the number porting process is complete.

Please complete the following information as it appears on your telephone bill, a separate form will need to be completed for each location. for scheduled transfer dates please allow 10 business days.

By signing below, I authorize Ring Beyond and their agents to act in my behalf for the purpose of porting the local or toll free telephone numbers listed above away from my current carrier and change Responsible Organizations. I authorize Ring Beyond and their agents to obtain billing information, customer service records, and other network information required to provide me with service as a Ring Beyond VoIP customer. I authorize Ring Beyond and their agents to act as my new primary service providers and discontinue service, cancel services, or make changes to the account as necessary with our current providers. I understand that if I am currently under a contract with my current service provider that cancelation fees may apply. Please also attach a recent copy of the full telephone bill.
